USB Loopback Errors

We are seeing the following USB Loopback Error using Passmark 5.2, USB 1.1 Loopback Plugs and Windows 2000; USB, SERIOUS: USB1 Plug 1, Data packet transmission failed (Code 4) [Serial#; PM29KDT6]. Is this an internal data packet transmission fail that gets corrected by CRC in the USB or a real data packet error?

This is a I/O write error, rather than data validation or CRC error. It is a real error that would not be corrected in a lower USB layer.
